Requirements For  MRP Passport Application

First Time Applicant
  1. Personal appearance of applicant is required except when the applicant is 8 years old and younger for data verification, thumb mark/finger –printing and signing of the computer generated application form.
  2. Four (4) recent  photographs (45mmx35mm) taken within the last 6 months with royal blue background, 70 -80 % image size of the applicant  in matte paper showing him/her in decent  attire.
  3. Birth Certificate (GEBURTSCHIEN) - 3 copies and original (If the original is returned to the applicant,  the 3 copies of the Birth Certificate will have to be certified “ Original Seen ” by the Embassy ,. and for this service, the consular fee is Sfr.35.00. If the original copy is submitted, there is  no fee required.
  4. One (1) copy of parent’s marriage contract.
  5. Photocopy of passport page with personal data of Filipino parent.
  6. Duly accomplished passport application form (FA form No. 1). Forms may be  obtained at the Embassy
  7. Duly accomplished Report of Birth (FA form No. 40) - three (3) copies) Forms may be obtained at the Embassy.
  8. Copy of Residence Permit (Bewilligung)
  9. Self addressed envelope with Sfr.5.50 stamp. (Registered) To be used in returning processed documents.

  • Sfr.  70.00     New Passport    (32 pages)
  •        84.00     New passport   (64 Pages
  •        35.00     Report of Birth fee
  •        35.00     Original Seen/Certified true copy
Passport Renewal
  1. Personal appearance of the applicant is required except when the applicant is 65 years old and older for data verification, thumb mark/finger –printing and signing of the computer generated application form.
  2. Photocopy of passport ( first 3 pages of brown passport/first and last page of green passport)
  3. Four (4) recent  photographs (45mmx35mm) taken within the last 6 months with royal blue background, 70 -80 % image size of the applicant in matte paper showing him/her in decent attire.
  4. Duly accomplished passport application form (FA  form No. 1) Forms may be obtained at the Embassy
  5. Copy of residence permit (Bewilligung) or Swiss passport,   if holder of a Swiss passport.
  6. Self addressed envelope with Sfr. 5.50 stamp. (Registered) To be used in returning processed document.

Replacement Of Lost  / Destroyed  / Tampered Passport
  1. Personal appearance of the applicant is required for data verification, thumb mark/finger –printing and signing of the computer generated form.
  2. A sworn statement narrating the circumstances on how the passport was lost, damage or destroyed.  (to be executed at the Embassy)
  3. Police report, in case the passport was lost.
  4. Four (4) recent  photographs (45mmx35mm) taken within the last 6 months with royal blue background, 70 -80 % image size of the applicant  in matte paper showing him/her in decent  attire.
  5. Duly accomplished passport application form (FA form no. 1) to be obtained at the Embassy.
  6. Copy of residence permit (Biwilligung) or  Swiss passport, if holder of a Swiss passport
  7. Self addressed envelop with Sfr.5.50 stamp. (Registered) To be used in returning processed document.
  8. Passport issued before 1990, or if pertinent information will be difficult to obtain, applicants must submit requirements for first-time applicants, such as Authenticated Birth Certificate, secondary identification and other supporting documents as may be required by the Consular Officer.

  • Sfr. 126.00  Replacement of lost passport  (32 Pages)
  •       196.00  Replacement of lost passport  (64 pages)
  •         35.00  Sworn statement of lost passport
  •         20.00  Cost of cable verification
Note:

Mode of payment: Consular fees should be made through postal order (Post Anwiessung) addressed to Philippine Embassy. Personal check or stamps are not acceptable as payment for Consular fees.
